Revised Procedure for Obtaining a Library Card

Beginning July 1, 2002, two pieces of identification will be required, including one that identifies who the applicant is and his/her address. The second piece of identification will also be required to show the address.

Acceptable forms of identification include: driver’s license, state ID, checkbook, voter registration, utility or other bill, rent receipt, postmarked mail, delivery papers, Medicaid or other social services documentation, tax forms, paychecks, and other family members already in the library card database.

Children may obtain a library card, after completion of an application, with two items of identification presented as proof of a current address. Having a parent in the library database may serve as one form of identification. A child under the age of 13 needs the signature of a parent/legal guardian, and both the child and parent must sign the library card.

People who are paying for a temporary card will need one form of identification that verifies identity and permanent address and one that shows a temporary local address, such as a Mill Mountain Theater ID or letter from a hotel or motel. These borrowers are limited to two items at a time.
